FosterVision
FosterVision is Santa Clara County’s secure, interagency data system designed to improve coordination and outcomes for youth involved with public systems. The platform integrates and updates information daily from all county school districts, Child Welfare, and Juvenile Justice, creating a comprehensive, real-time view of student status, placement, and service involvement.
By bringing together data that was previously siloed across agencies, FosterVision enables authorized staff to quickly identify youth who may be at risk of academic disruption, placement instability, or unmet service needs. This timely visibility allows educators, social workers, probation officers, and support providers to collaborate more effectively, respond proactively, and ensure that youth receive the appropriate educational and supportive services without delay.
Through this data-sharing partnership, Santa Clara County strengthens cross-agency communication, reduces gaps in support, and promotes stability, well-being, and academic success for foster youth and other system-involved youth receiving services from multiple agencies..
Service Link
Service Link is a collaborative initiative between the Santa Clara County Office of Education (SCCOE) and the Santa Clara County Behavioral Health Services Department (BHSD) designed to strengthen coordination, referral management, and service delivery for students and families. The platform provides embedded school coordinators with secure, role-based access to relevant student data housed within the county’s data warehouse, enabling them to efficiently log referrals, document outreach, and track family engagement and support activities.
By centralizing referral and engagement information in one system, Service Link streamlines communication between schools and behavioral health providers, reduces duplication of effort, and ensures that students are connected to appropriate services in a timely manner. Coordinators can monitor referral status, document follow-up actions, and maintain a clear record of services provided, supporting continuity of care and more effective case management.
In addition to supporting day-to-day service coordination, Service Link generates de-identified, aggregate reports that allow SCCOE and BHSD to monitor program implementation, assess service utilization trends, and evaluate the effectiveness of the Behavioral Health Services School Linked Services initiative. These insights help inform program improvements, guide resource allocation, and strengthen the county’s ability to deliver responsive, data-informed behavioral health supports that promote student well-being, engagement, and academic success.
